SHIIP data contributions provides access to patient encounter data across many health care sectors in real-time or near real-time.

Providers can view encounter data to confirm patient engagement with the health sector in real-time. -

The inclusion of encounter data provides a comprehensive view of the patient’s health care journey.
Contributions are received from acute care, post-acute care, home and community care support services, addictions and mental health, community support services and primary care (coordinated care plan) sectors.
Data Integration Details
With secure access, primary care providers can contribute data via Coordinated Care Plans, maintain their practice lists, and add applicable diagnoses to the patient record.
SHIIP receives hospital contributions from all 5 acute care sites and 1 post acute care site located in the southeastern geography of Ontario Health East and 1 acute care site in the Champlain region. Hospitals contribute to SHIIP ADT messaging in real time and DAD and NACRS information on a quarterly basis.
Admission, Discharge, and Transfer (ADT):
Captures patient demographic and administrative information when a patient is admitted, discharged, or transferred.Discharge Abstract Database (DAD):
Captures administrative, clinical, and demographic information on discharge. This includes deaths, sign-outs, and transfers.National Ambulatory Care Reporting System (NACRS):
Supplies SHIIP with valuable hospital-based and community-based ambulatory care data. This includes day surgery, outpatient / community-based clinic data, and emergency department data. This data is collected at the time of service, but methods of this collection may vary between facilities.Combined, the following hospital data is supplied to SHIIP:
- A patient’s identifying information (names, health card number, date of birth, etc.)
- A patient’s attending / referring / consulting physician
- Patient visit details, admission type, location, service
- The patient’s location (floor, room, bed) if available
- Admission date/time, discharge date/time
- Patient’s primary care doctor
- Patient’s chief complaint and other issues
- Any diagnoses codes assigned to the patient
SHIIP receives Home and Community Care data from the Home and Community Care Support Services located in the South East geography of Ontario Health East in real-time. This includes:
- Referral details
- Service details
- Medical equipment and supplies
- Safety details
- Assessment details
SHIIP populates patient records with community support data in real-time. The following information is captured:
- Patient details (name, address, phone number, etc.)
- Service name
- Service notes (if available)
- Service start / end dates
- Assessment scores (AUA, CSI, MAPLe)
SHIIP populates the patient record with services provided by Addictions and Mental Health (AMH) agencies in real-time.
